Form: Administration of Medication

In a nutshell our policy/procedures is as follows:

We administer prescribed medication which cannot be managed around the school day - this is predominantly medication which needs to be taken four times daily. The form below must be completed before we can administer medication.
We administer non-prescribed medication which supports a long term medical condition.
